Might be worth noting I had a couple of issues as I had to delete the .mp3 AND .aac version of the file AFTER the copying and renaming to .m4r to the file on my desktop.
As soon as I cleared these two files and double clicked the .m4r file on my desktop, it slotted nicely in to the ringtones section of iTunes.
